  3. Owner's Identity Card: A valid Emirates ID of the owner and/or partners
  4. Service Description Letter: A letter detailing the services the healthcare facility intends to provide
  5. Power of Attorney: If an authorised person is acting on behalf of the owner, a legal authorisation document is required
  6. Facility Layout/Floor Plan: A detailed layout or floor plan of the proposed facility
  7. Trade Licence: A copy of the existing trade licence, if applicable

For the Final License of a Healthcare Facility, the following additional documents are needed — 

  1. List of Healthcare Professionals: A comprehensive list of all healthcare professionals to be employed at the facility

  5. Preliminary Registration with FANR: If the facility offers radiological services, preliminary registration with the Federal Authority for Nuclear Regulation (FANR) is required
